Create New File

Enter the file information and press Create File. You will be redirected to the File Editor page, and start entering the file data.

File Name

The file name is a required field. A good file name will allow you to search and locate this file.

File Type

For pre-decree cases, the asset data entry form will include fields to furnish pre-marital value. The printed form will include an extra field for each asset on the asset list to show the pre-marital value and list it as an asset under the separate assets section.

For Post-Decree or APR cases, you can remove the Separate Assets section, and enter assets under the appropriate asset category. You may also omit the entire Assets section from the data entry form and the printed Sworn Financial Statement.

Parties

The party information is always required.

The Affiant is the person filing the Sworn Financial Statement. Select the party the Sworn Financial Statement is prepared for, and enter the names of both parties.

Select Both Parties to prepare the Sworn Financial Statements of both parties in a single file, for example in a mediation or an uncontested divorce. Employment, income, deductions and expenses are entered for each party, while assets and debts are entered once and shared. When printing, you choose which party's statement to print.

For Pro-Se accounts, select which party you are. Your own name is taken from your account information. The Both Parties option is not available for Pro-Se accounts.

Once the file is created, the party the Sworn Financial Statement is prepared for can be switched to the other party from the Print Statement page. A file prepared for both parties stays that way, and a file prepared for one party cannot become a file prepared for both.

Without a subscription, the party information is locked once you pay for the file. Make sure you enter the correct information.

Case Details

This section is optional with an active subscription. Without a subscription, the case details are required, and are locked once you pay for the file. Make sure you enter the correct information.

Enter the case number and case type. If the case type is Parental Responsibilities, enter the children names as well. Select the court from the list. Optionally, enter the division and courtroom.

Representation

This section is only available for law-firms.

If you represent the affiant, select the affiant from the list. If you are a mediator, select the Mediation option. The third option, the Other Party, should not be used except in very special cases, and is likely to generate a Sworn Financial Statement with some inconsistencies. If you represent the other party, you probably need to switch the parties.

When the file is prepared for both parties, the Affiant option stands for whichever party a printed statement is for, and the Other Party option is not offered.

Select an attorney or LLP from the list. You can select more than one attorney. For mediation, select Party Without Attorney, and enter the party's contact information. If you need to add an attorney, press the Add New Attorney or LLP button. You may also update attorney information by clicking the Attorney and LLP Management link. However, adding or modifying attorneys is only available for account administrators and managers, so if you don't see these options, contact one of your account administrators for assistance.

File Permissions

Use the File Permissions option to share the file with all account members or with your group members, or to keep it accessible only to you. This option is shown only when your account has more than one applicable choice.

Description

Use the description field to save additional information about this file. The field content is not printed on any form. Use it for information you don't wish to include in the file name, but might use when searching for this file.
